Maryland Self-Directed Care For Adults With Developmental Disabilities Announced

In Montgomery County, MD, Community Health and Education Services (CHES) (240-755-5591) has announced its nursing support services for individuals with developmental disabilities who can self-direct their care plans.

With this announcement, CHES gives individuals with disabilities two main options for their self-directed care, depending on their level of need: nursing consultation or nursing case management and delegation. On its website, the organization offers advice for choosing the most appropriate option by asking clients guiding questions like "Do you take responsibility for managing your meds yourself?"

CHES says the nursing consultation service is suitable for individuals who have a Health Risk Screening Tool (HRST) level of 3 or higher and are generally capable of self-directing their day-to-day care. This service includes a self-administration assessment and a comprehensive nursing assessment done by a Registered Nurse who has completed DDA-approved RN training. Individuals with developmental disability, spina bifida, cerebral palsy, or autism receive help creating their healthcare protocols or routines, assistance developing methods for communicating with doctors, and up to four visits from a CHES nurse per year.

The organization's Nursing Case Management and Delegation service is appropriate for individuals with any HRST level but tends to be more suitable for those with higher levels. As part of the service, CHES provides a registered nurse who issues all necessary assessments, develops the individual's care plan with them, and trains staff to complete all pieces of the care plan. This option comes with in-person nurse visits at least once every 45 days, but more if needed.

Individuals interested in self-directed, at-home care can contact the team at CHES to discuss which option is most appropriate for their needs and goals.
